My friends’ international flight leaves at 8am… It means she has to be at Heathrow at 5am, so she should be leaving the City at around 4am. Are there any public transportation services running at that time? Or is she stuck with an Uber/taxi? Thanks!

Depends on the day of the week…
The tube/subway does run 24 hours now on some lines including the Piccadilly line to Heathrow. THis is only on Friday night/Saturday morning and Saturday night/Sunday morning though.
Other than that there is a Heathrow Express train which leaves Paddington Station from 5.10am and arrives at 5.25 - should still be plenty of time for an 8am flight. https://www.heathrowexpress.com/timetable-schedule/london-heathrow
There is also a network of buses that run through the night but to be honest its not that great or reliable - fine to get you to Paddington but I wouldn’t want to go all the way to heathrow on them.
