Yes that’s correct @JoelBarrett . Just to clarify and for everyone’s benefit, there are two requirements for being included (1) you have submitted your entry form which can be confirmed by you revisiting your form and you can make edits to it, and (2) you have completed your confirmation of accepting the rules etc. which is important because you will be providing me with your personal information for use on this new project. It’s a good idea to re-read the information in the updated post and make sure that you understand what this project is all about because, as I have said there, this is more than just about points. By the way Joel, yes you have completed both (1) and (2) mentioned above 
If you mean the first Beta Testing @JoelBarrett then yes I see that you have successfully submitted that and I will hopefully see your answers here as others like @AjitThite @Robert24 @Cercis and @@Chacho have don.

1 - Was the form easy to follow, understand (Yes/No) ?
For a tech person, yes. For a tech-adverse person, they would be lost just reading the requirements.
2 - Did you have any problems? If so describe each of the problems.
Not really, just had to use separate screens. One to show the form request and one to show the webpage I was looking for answers in.
3 - Were there any parts or words that were unclear. If so which ?
No
4 - Was it clear what you had to do at each step (Yes/No) ?
Yes, for me.
5 - Unlike Beta Test #1, you will note that in Beta Test #2 there were few pictures. Do you think pictures like we had in Beta Test #1 would make things clearer?
Yes.
6 - Do you have any suggestions on how to improve this?
More pictures.
7 - Any other comments?

Yes. However, you need to be focused in order to be error free. More errors in the submission expected.

I didn’t face any problems.

No

Yes.

@Chacho thank you again for getting back to me when you did and letting me know that the link worked. Because of problems some had with Beta Test #1, I didn’t want to disappear until I knew that you guys could continue, so again thank you. I wait to see your responses to the 7 questions in a comment here on the post.
@JoelBarrett thanks for being first cab off the rank with your response to the 7 questions and in particular, for following the format I asked for and sets an excellent example for others.
@Kahnster thanks I see you finished which is fantastic.
So now I hope to get the final responses from @Robert24 @Cercis and @AjitThite soon. In the meantime can you each tell me:
8 - What device did you use to complete the test (Desktop PC, Laptop computer, Tablet or Mobile phone)? Joel you mentioned two screens so I assume you did it on desktop.
9 - How long did it take you?
We will move to the final Beta Test and very soon and wrap up this stage of this project up fairly quickly.
I will provide further comments on your Beta Test #2 responses a bit later.
