However, from you contributions I see that you tried to add some place already, so you already know the procedure.
From the photo you added, it seems you are trying to add your own business. Please keep in mind that the Local Guides Program is not for businesses.
This is against the Local Guides Program Terms and Conditions : “Organizations, brands, and businesses are not eligible for the Program.”
If you want to add your own business, use the free program of Google My Business, so please, that is specific for Businesses in Google Maps.
Follow this link: https://www.google.com/business/. With My Business you can also have more control of your listing in Google Maps.
