Temple No.12 was built in two phases which is attractive and grand from the art and architecture point of view of this temple. The new construction was done on the old remains according to the same plan.
The exterior of the earlier temple is decorated with pillars, decorative moldings and niches to keep stucco images, while the latter is mostly plain. This huge east facing temple was built on a square platform.
It was a temple built in Panchayatana style in which the main temple was built in the center and subsidiary temples were built in the four corners. The traces of which are visible on the platform. It can be reached through a flight of stairs. Its first phase corresponds to the fifth phase of the temple on the platform. On the basis of the decorative work and architecture of this temple, it appears to be of the 6th century. There are many votive Stupas in the proximity of the temple number 12.
