In Southeast Asia and many places around the world, it’s hard to find clean and accessible public restrooms. As a Local Guide, I think Google Maps should allow us to rate restroom cleanliness, tag availability, and highlight accessible toilets.
This small update could help families, travelers, the elderly, and people with health needs plan better.

There is already a category specifically for accessible public toilets, @SophatCHY.
I think people will comment on the cleanliness in their review texts. Or how would you like this improved?

Google Maps already provides categories to help you understand whether restrooms are Wheelchair Accessible 
. As your @MortenCopenhagen Bhai mentioned in the comment, I agree with him.
We, local guides, can do this by finding all the accessible toilets in the city and compiling a list that will be open to everyone. Everyone can benefit from it.
Just like I started a campaign in my country with my local guide friends.

Campaign 3: Upcoming.
We will be able to learn how clean these toilets are from the reviews given directly by those who use them.
